Policies

Practice Policies and Information:

Fees:

Self pay rates are as follows:

Initial Assessment: $200

Individual Therapy Session: $135

Insurance:

I am currently in network with Blue Cross Blue Sheild and Aetna/Aetna State Health Plan as Clear Choice Provider.

Payment is due at the time of services and is accepted by cash, check or credit card.

More on Insurance:

Please know that the rate of coverage for sessions is ultimately determined by your specific insurance plan.

Therefore, I encourage each of my clients to reach out to their insurance directly as insurance will give more detailed information to the member/insurance policy holder (you) than their providers (me). This is the best way for you to have the most accurate information.

Before our first appointment, please call your insurance company to confirm if I am in network, learn about your deductibles and copays for mental health services for both in-person and telehealth services.

It is the client responsibility for payment in full for services. This includes whatever balance remains in full after insurance after claims processing.

Out of Pocket Information:

If you elect to pay out of pocket and wish to use out of network benefits I can provide you with a document called a “Superbill” to submit to your insurance provider to request reimbursement upon request.

You are entitled to a Good Faith Estimate under the No Surprises Act if you plan to self-pay for sessions.

Late Cancellation Fees:

I have a 24-hour late cancellation/no show policy. There is a $60 fee that will be charged to the client’s credit card on file for late cancellations/no shows not cancelled 24 hours prior to the session start time. Please know this fee not reimbursable by insurance.

Other Policies:

We will review a more extensive list in our first visit when we go over my Professional Disclosure Statement. However here are a few important things to know prior to beginning our work together. I believe that having healthy and professional boundaries are an essential part of our work together so that you can get the best care and make the most progress.

  • I am in the office Monday – Thursday from 9am-2pm. I am unavailable after hours and on weekends and do not provide crisis services. If you are experiencing a clinical emergency or are in crisis please refer to the emergency resources page.

  • I do take periodic time away from the office where I will be unreachable for 1-2 weeks at a time.

  • My professional code of ethics discourages dual relationships. Therefore I do not accept friend requests on social media.

  • If we ever determine that my professional skillset is not within the scope of your needs, I will discuss referrals to another therapist so that you can get the care you deserve. Additionally, our first few visits also serve as us getting to know each other and determine if we are a good match for your concerns and goals.

Other Service Limitations:

The following items are outside of the scope of my practice:

  • I do not provide forensic services such as custody evaluations, assessments recommended by court, probation, etc.

  • I do not provide Psychological Testing services such as for ADHD, Autism, or Learning Disabilities.
